Analyzing the understanding of pharmacy students about the severity and the resolution of medication errors causes

Abstract
Objective: To assist the Pharmacy students improve their understanding and proactively learn, find out the causes of errors in medication use and how to fix those errors.
Subjects and method: Pharmacy students of course 3 and course 4 - Vietnam Academy of Traditional Medicine and Pharmacy have learned credits in Clinical Pharmacy and Drug Use. From January 2022 to June 2022 at Vietnam Academy of Traditional Medicine and Pharmacy. Cross-sectional, qualitative and quantitative descriptions.
Results: For patient-related factors, the results of the severity of the cause of errors were from 3.09 to 3.53. With causes of errors related to patients at level 3 and level 4 were accounted for 34.7% - 56.4% and 24.8% - 40.6%, respectively; For factors related to medical staff, the results of the severity were at level 3 (accounting for 36.6% - 49.5%). Students have the correct awareness and understanding of measures to overcome the causes of errors in medication use, specifically: 31/39 solutions have a student answer rate of over 70%; 4/39 correct answers are below 34.7%.
Conclusions: Up to 34.7% - 56.4% of the research subjects assessed the severity of medication errors from both patients, doctors and drugs. Students correctly understood 31/39 of the solutions to overcome the causes of medication errors.
