Abstract

Objective: To analyze the current status of antibiotic (AB) utilization and stewardship in inpatient care at Kien An Hospital, Hai Phong, 2024.

Subjects and methods: Cross-sectional descriptive study, collecting data on AB use and management.

A cross-sectional descriptive study was conducted to collect data on AB utilization and stewardship.

Results: 91 AB items were used, total value > VND 24,5 billion; Beta-lactam was the most common group, accounting for 56.0% of item quantity and 78.16% of usage value (> VND 19.1 billion); ICU had the highest AB usage value (20,48%); Single-component ABs were more prevalent than multi-component ones; Injectable powder form accounted for 57.1% of item quantity and 79,68% of value; Generic drugs were used 43 times more than original brand-name drugs; Injectable/infusion ABs had the highest usage rate. The Antibiotic Stewardship Committee of the hospital was established in 2021 with full required members; Meetings were held only during incidents, not regularly; Prescription monitoring occurred upon feedback; AB usage tracked via software. Over 80% of AB prescriptions met standards; Training held only upon updates; Annual usage reports and biannual resistance monitoring conducted; Quarterly evaluations of AB appropriateness; Surgical prophylactic AB use ranged from 30-60%; IV-to-oral switch rate < 20%; Inpatient AB usage rate: 30-50%; Main challenge in implementing Decision 5631/QD-BYT: Lack of supervisory personnel.

Conclusion: Kien An Hospital has relatively complied with MOH regulations on AB use and management. However, regular committee meetings, enhanced monitoring, continuous training, and dedicated staffing are needed. AB usage should follow department-specific protocols with rational IV-to-oral conversion.
