Drug utilization pattern and direct treatment costs of patients undergoing surgery for benign prostatic hyperplasia at Kien An hospital, Hai Phong city from 2023-2025

Abstract
Objective: To describe drug utilization and direct medical costs of surgical treatment for benign prostatic hyperplasia (BPH) at Kien An Hospital, Hai Phong, Vietnam during 2023-2025.
Subjects and methods: A cross‑sectional retrospective study on 231 medical records with the diagnosis of BPH (ICD‑10 N40), treated with TURP from January 1, 2023 to December 31, 2025. Data were extracted from electronic medical records and hospital management systems, processed with Microsoft, Excel, analyzed using Chi‑square test.
Results: The mean age was 72.4±8.6. 63.92% had comorbidities (the mean was 2.3/patient). 110 drug items, total drug expenditure VND 733.7 million. Anti‑infectives accounted for 78.90% of drug costs; Beta‑lactams accounted for 66.64% of that group. Generics accounted for 99.23% of drug costs (p<0.01); domestically manufactured drugs 79.29% (p<0.05). Total direct medical costs VND 5.39 billion, mean VND 23.35 million/patient. Other services constituted the largest cost component (44.49%) and were entirely out‑of‑pocket, health insurance covered only 50.1% of total costs.
Conclusion: The study demonstrates successful implementation of generic and domestic drug policies, while revealing a substantial out‑of‑pocket burden from non‑covered services. Expanding health insurance coverage, strengthening antibiotic stewardship, and enhancing transparency of service charges are urgently needed.
